The Evolution of Kidney Graft Preservation Through the Years

In this review, the authors provide a comprehensive overview of the evolution of kidney graft preservation methods, detailing the transition from traditional SCS to more advanced technologies, such as HMP and HOPE. HMP has demonstrated advantages in reducing DGF and enhancing graft survival, with more recent studies advocating for the use of HMP across all donor types. LifePort Kidney Transporter was mentioned as part of the most widespread and commonly used HMP systems.

Other key points include:

  • HMP significantly reduces the risk of DGF when compared to SCS, regardless of donor type
  • 1-year and 3-year graft survival was superior with HMP compared to SCS
  • Therapeutic Hypothermia was found to be inferior to HMP in DBDs regarding DGF
  • Flow-driven devices require higher systolic pressure to maintain equal flow compared to pressure-driven devices such as the LKT, which produces endothelial damage and thus potentiating the risk of graft thrombosis

Evidence clearly situates HMP as superior compared to SCS, in both DGF and graft survival, independent of donor type.

Impact of Warm Ischemia Time on DCD Kidney Transplant Outcomes

This single-center retrospective review by Alghannam et al. looked at the impact of functional warm ischemic time (fWIT) in controlled DCD donors by comparing donor/recipient characteristics and outcomes between high fWIT (>60min), low fWIT (<60min), and DBD kidneys. Out of 2,811 patients from 2000 to 2021, 638 received low fWIT kidneys, 93 received high fWIT kidneys, and 2,080 received kidneys from DBD donors.

The results showed:

  • No significant difference in 5-year graft survival between low fWIT, high fWIT, or DBD groups, despite significantly higher DGF rates in both low and high fWIT groups
  • Kidneys with an increase in KDRI and high CIT had higher hazard ratios for 1-year graft failure

The authors noted that they acquired a hypothermic machine perfusion device in 2008 and routinely use it for all DCDs unless deemed unsuitable for logistic or technical reasons, which allowed them early expansion of DCD kidney transplantation.

They concluded that DCD kidneys with fWIT>60 min can be safely transplanted with careful donor and recipient selection and should not be declined solely for fWIT.

Cold Ischemia Time and Delayed Function in Kidney Transplantation: A Paired Kidney Analysis

This retrospective cohort study by Husain et al. analyzed US registry data. Investigators identified kidney pairs from the same donor where there was a notable difference in CIT between the kidneys.

The results showed that:

  • Out of 25,831 kidney pairs, 71% had concordant DGF outcomes, 16% had only the longer-CIT kidney with DGF, and 13% had only the shorter-CIT kidney with DGF.
  • Longer-CIT is associated with increased risk of DGF, even for small differences in CIT.
  • Longer-CIT kidneys that were pumped were less likely to develop DGF than shorter-CIT kidneys that were not pumped.

The authors concluded that pumping kidneys may mitigate and even overcome the adverse consequences of prolonged CIT on the risk of DGF.
